Nadelin Disaster occurred on October 24, 1960, when a Soviet R-16 rocket blew up at the launch pad. It was an accident that incinerated nearly 72 military personnel including the chief Marshall Mitrofan Ivanovich Nedelin, the head of Russian rocket programme. The reason found out was the short circuit in the replaced sequencer that ignited the first stage fuel tanker placed right below it, destroying the missile completely and incinerating people standing close to it.
Apollo 1 or AS-204, was the first manned mission of NASA’s Apollo programme. But, the miserable tragedy during launch rehearsal test on January 27, 1967, put this lunar landing plan to halt. The crew member included the commanding pilot Virgil Ivan "Gus" Grissom, senior pilot Edward Higgins White, II and pilot Roger Bruce Chaffee and all three of them died in that accident. As soon as they entered the cockpit, they started facing problems. And, a minute later, they complained about the fire in the cockpit. In less than 17 seconds, all of them died by suffocation caused by Carbon Monoxide gas. The reason for fire is still unknown.
This was the first accident where casualties happened in space. On June 7, 1971, Soyuz 11, a Russian spacecraft became the world’s first mission to board the space station. After completing 22 days stay at the station successfully, when the spaceship departed on June 30 with the crew members- Georgi Dobrovolski, Viktor Patsayev and Vladislav Volkov, a critical valve exploded thirty minutes prior to landing. Due to this, all of the oxygen was released into space. All the astronauts died in less than a minute because of high-altitude decompression.
This tragedy took place on February 1, 2003, when a piece of foam insulation disintegrated from the external tank of the space shuttle at the time of the launch. When the spacecraft re-entered in the atmosphere, the hot gases penetrated and destroyed the internal wing, thereby destabilizing and breaking the orbiter apart. In this fatal accident, all the seven crew members died. The team included NASA's best astronauts, namely Rick D Husband, William McCool, Michael P Anderson, David M Brown, Kalpana Chawla, Laurel B Clark, and Ilan Ramon.
